Disease-free Origin Certificate System (Pink Forms)
The export of food products, vegetables, and fruits to international markets requires a Disease-Free Food Production and Origin Certificate, commonly referred to as Pink Forms. This certification is issued by the Department of Disease Control, Ministry of Public Health, to provide confidence to importing countries that food products meet the required quality and safety standards and are free from cholera-related health risks.
The TEMS Pink Forms System has therefore been developed specifically to support small and medium-sized exporters who have a relatively low volume of data submissions and do not require the installation of a licensed software program. The system enables direct electronic data exchange with the NSW Service Provider (NSP) through a convenient web-based application. Users are not required to obtain or install a Certificate Authority (CA) for data transmission, helping to simplify the process and provide greater convenience and accessibility for users.
Registration Process
1. Access the website at: https://pinkforms.kainum.com
2. Download the TEMS Pink Forms System Registration Form from the Login page.
3. Complete the Registration Form and submit it together with the required supporting documents listed below to temspinkforms@gmail.com.
3.1 P.P. 20 (VAT Registration Certificate) The document must be signed by the authorized signatory in accordance with Clause 3 of the Company Registration Certificate and affixed with the company seal.
3.2 Company Registration Certificate The certificate must have been issued within the last three months. It must be signed by the authorized signatory in accordance with Clause 3 of the Company Registration Certificate and affixed with the company seal.
3.3 Registration Application Form The form must be signed by the authorized signatory in accordance with Clause 3 of the Company Registration Certificate and affixed with the company seal.
3.4 Copy of the National ID Card / Passport of the Authorized Signatory For individual applicants, only a copy of the National ID Card is required.
3.5 Power of Attorney If applying as a Customs Clearance Agent, a Power of Attorney must be provided by both the exporter and the Customs Clearance Agent.
- If the company's authorized director is a foreign national: A Power of Attorney must be issued to one Thai employee of the company, together with a copy of the foreign director's passport and a copy of the Thai employee's National ID Card.
- If authorizing Digina Co., Ltd.: The Thai employee of the company must issue a Power of Attorney to Digina Co., Ltd., together with copies of the National ID Cards of both parties.
- Company seal requirement: All Powers of Attorney must be affixed with the company seal.
4. Registration Verification and Account Setup Our staff will verify the submitted information, complete the required account linkage and registration process, and send the Username and Password to the email address provided during registration.
5. Add Credit to the System Add funds to your account to cover the data transmission and receiving service fees.
